Summer Camp Forms
Wayfinder forms for parents/guardians are all digital and need to be completed online. The new OTC (Over the Counter) form is for you to print and bring to your doctor to sign and is required for all over-the-counter medication you want to send with your child. It is okay for your doctor to add prescriptions to the form as we also need a note from the doctor authorizing the taking of prescription medications at summer camp. Documents from your doctors can be faxed to (845) 481-5577, mailed or emailed to us. (Photos are also acceptable but scans are preferred.) These include the “Child Health Assessment Form*”, immunization records or medical exemption letter, a note authorizing your child to take the medications being brought and original prescriptions. Please also remember to bring the original bottles medications came in for both prescription and over the counter medication.
*The “Child Health Assessment Form” is called different things in different doctor’s offices. It is a New York State requirement for summer camp and can be met with the NY State form required for public school: http://www.p12.nysed.gov/sss/documents/health-exam-form.pdf. It must be less than one year old from the start of camp. If your doctor is not familiar with it, a note that says your child is well enough for summer camp is sufficient. Please note we need all the forms below for overnight summer camps. Day campers do not need the Overnight Camp Covid Test Consent Form.
Please note that ALL MEDICATIONS require a doctor’s written prescription or doctor’s order, even over the counter drugs. You can use our OTC form for this purpose. The form is intended for printing and bringing to your doctor.
